Does this mean there are times when it is not appropriate to go to confession?

Fr. Dave mentions people not believing something is a sin and not having a desire to change from sinful ways. Based on the stated example of premarital sex does this mean that if I examined my conscience, prayed, understand the churchs teaching but still conclude that premarital sex is not a sin, that it is not a sin for me? Do I not confess this one sin or do I not go to confession at all?
It seems ironic that confession excludes the very people that may need it the most. What do you do when you struggle with faith and your conscience is not aligned with catholic doctrine?
Fr Steve also discussed being penitent. What do you do if I feel guilty for sinning but lack sorrow?

How have other people worked thru these struggles while respecting the Church?

    Thank you,
